Employment Application

Ave Maria Village offers many opportunities for individuals in the health care field.  Positions are available for registered and licensed practical nurses, medication assistants, certified nursing assistants, housekeeping, laundry, dietary, and more.  There are a variety of exciting career possibilities available in a number of different areas.

Benefits

The purpose of providing benefits is to address the economic and security needs for you and your family.  In some instances, benefits may vary among facilities, but only when SMP Health System has consciously decided that differences should exist for good business reason, usually related to different markets and competitive practices.  As well, some facilities have employee classifications that are not eligible for benefits unless stated in the policy for the benefit.

Employees working at least 16 hours per week are eligible for the following benefits, and benefits are effective the first of the month following your date of hire:

  • Health Insurance – Blue Cross Blue Shield – Select Choice
  • Dental Insurance – Blue Cross Blue Shield
  • Vision Insurance – Blue Cross Blue Shield
  • Life Insurance – One times your annual salary is paid by AMV.  You can purchase additional life insurance in $10,000 increments according to the current premium schedule.
  • Dependent Life Insurance – Coverage for your legal spouse in the amount of 50% of your additional life insurance amount and for your eligible children in the amount of 10% of your additional life insurance amount (maximum $10,000) is available according to the current premium schedule.
  • Accidental Death & Disability Insurance – One times your annual salary is paid by AMV.  You can purchase additional AD&D insurance in $10,000 increments according to the current premium schedule for yourself, your spouse, and your children.
  • Flexible Spending Accounts – Provide you with the opportunity through payroll deduction to set aside a portion of what would have been taxable income and deposit the money into a personal reimbursement account for Medical and Dependent Care expenses.

Paid Time Off

A time off program for eligible employees working at least 16 hours, pro-rated on regular hours worked and your length of service.

Disability Program

A serious medical condition can result in the loss of income that is important to you.  The disability plans administered by SMP Health System will help to provide protection during such an event.  The disability program is available after one year of service when working 24 hours per week.

All employees are eligible for these benefits
  • Employee Assistance Program (EAP)– Through APS Healthcare, SMP Health System provides a valuable benefit to you and your family members by making independent and objective counseling or referrals for personal or work-related issues (marital, family, legal, financial, emotional, chemical dependency, etc.) up to three visits per problem at no cost to you.

  • Short Notice Pay
  • Employee Christmas Party
  • Service Award Program – In recognition of service to SMP Health System, you will receive recognition after the following continuous years of service (3, 5, 10, 15, 20, etc.).  Each milestone in your service at SMP Health System will be recognized by your facility.

Retirement and the 401(k) Savings Plan

SMP Health System’s principal source for retirement income is the 401(k) Savings Plan, a matching savings plan in which SMP Health System makes a substantial contribution for each dollar that you contribute.

  • Start contributing to the plan right away.
  • Funds are vested right away.
  • Central Dakota Village matches up to 4% of your annual pay after your first year of employment.
  • Choose your own investment options.
  • Change deferral amounts or make a change in investment options at any time.
  • Loans are available.
Salary

Central Dakota Village reviews salaries in the area periodically to stay competitive.  A new employee’s salary is dependent upon experience and will be discussed during the interview process.

$.25/hour evening positions
$1.00/hour night positions

Education Assistant Program for Nursing Positions

Nurses – Ask about our Nurse Scholarships or Loan Forgiveness Programs.
CNA Training – Classes are available to become a Certified Nursing Aide.

* Please note:  Some employment classifications are not eligible for benefits such as Temporary, On-Call,  and Per Diem.
